Breathe Well Box Questionnaire
The Breathe Well Box is a monthly care-package that will include 5-8 hand picked mental, physical, and spiritual wellness products. We plan to utilize ethically sourced and environmentally friendly products, and we are also partnered with a non-profit who will plant a tree for each month of a customer's subscription (a maximum total of 12 trees annually per customer). The products included in the box will range from yoga attire to water bottles, CBD products to healing crystals, various forms of aromatherapy, and a variety of other wellness products. Each Breathe Well Box is valued at at least $100.00 retail, and we plan to charge $59.99 per month! Our goal is to reduce people's daily stressors at an affordable price point and aid them in living an all around healthier lifestyle.

To get a better idea about what you may receive on any given month being subscribed to the Breathe Well Box, here is a sample that we have put together:

In this questionnaire, we will ask you a variety of questions regarding the Breathe Well Box. We want to know what the community we will be targeting thinks about our concept, and get the feedback necessary to create the perfect product made for yogis by yogis.
