I accept full responsibility for my child’s own actions and consequences as a result of his/her online attendance of North York Mandarin Alliance Church’s Summer 2020 VBS program.

I understand that the camp will be held online this year and I am responsible for supervising my child during the entire time of their online live sessions. I further give consent that the online sessions with my child’s images may be recorded for child safety purposes. I also authorize NYMAC to use any photos or videos taken during the camp for church promotional purposes.

By checking the box below in this release form, I agree to release and hold harmless North York Mandarin Alliance Church and its staff and volunteers for any damage or injuries, physical or mental, as a result of participation in the aforementioned camp
