University Housing Guest Check In Form
THIS FORM SHOULD BE COMPLETED BY THE GUEST, NOT THE RESIDENT YOU ARE VISITING.

All guests, including but not limited to, non-LTU visitors, parents, commuting LTU Students, as well as LTU residents who reside in another residence hall on campus must complete this form before entering the residence hall they are visiting. 

As a guest, you must stay with your resident host at all times. Residents will be found in violation of the Student Code of Conduct if they are not with their guest at all times. 

For the safety of all residents and your guests, please complete the form each time you bring a guest into the residence halls.

After submitting this form, you will receive a copy of your response to keep for your records in the event of an incident, or if a University staff member requests to see your confirmation email which becomes your Guest Pass.
