A copy of the child(ren)'s birth certificate(s) and, if Catholic, baptismal certificate(s) are required.  For students in grades 1-8, copy of the most recent report card and standardized test results (e.g., MAP Growth) are required.  Submit these documents via email (in PDF format) to admissions@ckslex.org.
