This form will be used to apply for enrollment into the program at Woodland Park Academy. Please complete all portions of this form so that staff can take the next steps to get your child(ren) enrolled.  Woodland Park Academy will reach out to your student's current and/or former schools to obtain behavioral records in order to be considered for enrollment.

Please note that Woodland Park Academy will also need a copy of the student's birth certificate, immunization records or waiver, and a copy of the guardian's Drivers License or State I.D. These documents are required to be turned in before the student can attend school. Thank You!
