As an SGT Member, I (we) agree to the following responsibilities by checking the boxes: *

Respectfully attend 1 SGT lunch meeting per month to discuss waste reduction and environmental & sustainability issues. (Meetings DO NOT impact recess.)

Understand the rules regarding recycling and waste reduction, and do your best to follow them.

Perform visual checks of RECYCLE and WASTE bins for any errors. If errors are found, alert proper personnel.

Lead by positive example and encourage others to care for our common home.

Field Trips are "optional" but if available to attend, I agree to display good behavior in line with St. Anne's Discipline Code.

Media Release Request

I hereby authorize and give my full consent to Shrine of St. Anne School to use, reproduce and/or publish any and all photographs, videotapes, and any other form of audio/visual communications in which my child appears while participating in the SGT program and activities related to the SGT program. I further agree that SGT may use these communications for any publications, public displays, exhibitions, web sites and other related endeavors without limitation or reservation. *
