SCSD is working to create a system where registration information can be submitted online, eliminating the need for repetitive paper forms.  Thank you for participating in this process.

This form is for NEW students only.  If you are registering a student from another SCSD school, do NOT use this form.  If you are currently attending another SCSD school and want to transfer to Midway, you must get approval BEFORE registering.  Contact the Midway front office if you want to transfer.

Please fill this out only if you are the legal guardian of the student, and your student did NOT attend school last year in the Socorro Consolidated School District. You will be asked for information about living arrangements, transportation, siblings who attend school, and medical information. Some information is optional; other information (medical, contacts, and transportation) is required and must be entered every school year.
