Music Education Services Video Lesson Permission

During the Illinois "Stay at Home" order, Music Education Services would like to offer video lessons to help guide students in their practice. Student lessons will not receive a grade nor will they be recorded for any reason. It will simply be used as a tool to help our students continue progressing during the school closures. Also, for the safety of everyone, an adult must also be present for the lesson. Please fill out the following form to get your child signed up to receive video lessons with your band director.

Also, if you have more than one child in band, please fill out a separate form for each child.
