Kindergarten families,

Welcome to London Elementary!  Thank you for beginning the enrollment process for your kindergarten student. We look forward to a memorable and successful year.

To begin registration, please answer ALL questions below. Once your form has been submitted, stop by LES to sign your child's printed enrollment form so as to verify that all information on the form is correct.

Once we receive your enrollment information, we will call you later this summer to further discuss registration, provide an official start date as well as provide open house dates and times.  

All required documentation (listed below) as well as a guardian signature on the printed enrollment form must be provided to London Elementary by July 31, 2020.

*Required Documentation

Families are required to provide the documents listed below to London Elementary to complete the full registration:

-Certified Birth Certificate
-Current KY Immunization Certificate
-Current KY Eye Examination (Completed by Jan 1, 2020)
-Current KY Dental Exam (Completed by Jan 1, 2020)
-Current School Physical (Completion date between August 6, 2019- present)
-Proof Residence. Acceptable proof of residence documents include: current utility bills (gas, electric, water). No final or disconnect notices accepted. Cable, internet, and phone bills are not accepted.
