Dear parents, caregivers and swimmers,

Kindly complete this form to book a Make-Up Lesson for a missed lesson in the aquaDucks swimming programme.

For lessons cancelled by students:
• Students are entitled to one (1) make-up lesson per month
• Please provide us with prior notice before the lesson time if your child is unable to make it for their regular lesson

For lessons cancelled by aquaDucks due to bad weather conditions:
• All lessons cancelled by our swim school due to weather will be entitled to make-up lessons.
• Valid until the student resigns from the aquaDucks swimming programme

Your venue coordinator will contact you within two (2) working days to arrange the make-up lesson.
