Thank you for participating in the 2021 P.L.A.Y. Shot Club. We want to support all our K-8 players in working on their basketball game throughout the summer.

Each month, please submit this simple form for each player in your family who is participating in the Shot Club. Simply fill out the player's name, month, and number of shots attempted that month. You will use this same form to submit each player's total each month.
