You will be required to complete this online health declaration before you or your children can access the Emanuel School Premises.

You will also need to complete this on behalf of your children who are attending Activ Camps. If there are other individuals collecting or dropping off your children, then you will need to complete the Authorised Persons section of this form.

Attendance eligibility (for indoor settings):

Prior to attendance we also require all parents and carers to have read the relevant government guidance for providing indoor provision at this time.
