Please fill out this form for EVERY gym that you would like games scheduled out.
Please fill out a new form for each gym.  Only one gym listed per form.
SCHEDULES CANNOT BE RELEASED UNTIL ALL OF THE LOCATIONS ARE ENTERED INTO THE SOFTWARE.  PLEASE EXPEDITE THIS REQUEST!

School Name = Name of your sponsoring organization (Gresham, Lake Oswego, Sherwood, etc)

Scheduler Info = Name and contact information of who will be responsible for your scheduling this season.

Gym Name = Official Name of gym including if gym is "small gym" or "Gym A" to differentiate locations.  There will be a google address linked to each location so please give official name of gym so location is correct.
Availability of Gym = All dates & times that gym will be available for a game to e scheduled.  it is okay to list them as specific dates, and it is okay to say "Tuesday & Thursday from 6pm to 8pm and Saturdays from 8am to 3pm".  Whatever works best.  

Grades & Gender for each Gym = Please indicate what level of play should be scheduled in each gym.  4B= Fourth Grade Boys.  7G = Seventh Grade Girls. Etc.
