CCP Declaration of Intent 2021-2022
This online form serves as the State of Ohio's Declaration of Intent form for participation in the College Credit Plus program with the Pickerington Local School District.  Please review and agree to all sections prior to May 1st 2021 and click submit to complete the form.

I declare my intent to participate in the College Credit Plus (CCP) program. I understand that: 1) signing this form does not require that I participate during the coming school year and may decide not to participate without consequence; 2) it is my responsibility to notify my school if I am not admitted to an institution of higher education or choose not to participate for some other reason; 3) I have received counseling about the program concerning the rules and regulations for both my school and the college; 4) I understand the responsibilities, the benefits and risks of participating in the CCP program.   *

CCP courses are part of student’s full academic record and may impact the student’s ability to graduate high school.   *

Completion of this form does not guarantee course enrollment; colleges determine course requirements and the state determines program eligibility. Course drops must be made prior to the college partner’s census date to avoid paying for the course.  Enrollment requires correct application to the college. *

The district may bill the family for course costs if the course does not yield earned credit.  The program requires a cumulative college GPA higher than 2.0 to remain in good standing for the CCP program. *

Course texts are the property of the district and must be returned.  Books independently purchased without written permission from the district treasurer are the student’s private property and not subject to reimbursement through the program. *

Students may be required to interact with classmates on group work and projects inside and outside of the classroom. The subject matter of courses may include mature themes and materials and will not be modified for students participating through CCP.   *

Students with documented disabilities must request a review of accommodations through the partnering college’s Office of Disability Services and should not expect the same level of accommodation in CCP courses. *

CCP courses funded taught on district property are considered a post-secondary under federal law.  Students (including minors) have all rights transferred to them alone.  Instructors cannot discuss student performance with anyone but the student. Educational records (grades/attendance) will be posted on Infinite Campus *

Communication about a student’s CCP courses or requests will be made via the student’s PLSD email account.  The district’s CCP webpage is accepted as final communication of changes made to the program.  For all academic issues, the partnering college’s policies and the course syllabus supersede district policy. *

Students must request official transcripts from the district’s college partner for credit to be formally transferred. *

CCP courses taken directly through a college that are offered in an online format may not be compatible with Chromebooks.  The district is responsible for instruction materials specific to course content but is not responsible for the acquisition of computer hardware. *
