After-School Enrichment Program (K3-12)
Enrollment into the After-School Enrichment Program (ASEP) is achieved by completing the form below. A one time enrollment fee of $30.00 will be charged with the first billing. This enrollment form is only required for full-time attendance.  Please see https://www.elcaonline.org/asep for a breakdown of ASEP fees.

Students attending ASEP on a drop-in basis should not be formally enrolled and will be billed according to attendance at $18.00 per day. All students not formally enrolled for full-time attendance are considered "drop-in." NO ENROLLMENT IS NECESSARY FOR DROP-IN ATTENDANCE.

BY ENROLLING INTO ASEP YOU UNDERSTAND THAT YOU WILL BE BILLED ON A FULL-TIME BASIS, ACCORDING TO THE BILLING POLICY OUTLINED AT www.elcaonline.org/asep, WITHOUT REGARD TO ATTENDANCE.  ONCE ENROLLED, A WITHDRAWAL FORM MUST BE COMPLETED TO CHANGE YOUR ASEP STATUS FROM FULL-TIME TO DROP-IN.

PLEASE COMPLETE A SEPARATE FORM FOR EACH STUDENT.

By completing this form, you agree that the weekly cost of ASEP will be added to your tuition statement as long as your child is enrolled, regardless of the number of days attended. If you wish to withdraw your child from the program, a withdrawal form must be completed to cease billing. There is a $30 fee to re-enroll after having with-drawn.

* $2.00 per minute is charged to the student account for pickup after 6:30 PM.
