Star Wars Day Drawing Entry Form
The Adams Memorial Library Children's Room is excited to celebrate Star Wars Day. We would love to see how YOU celebrate -- whether it's a costume, a story, a craft, or something else entirely!

Each participant who submits a photo of their Star Wars Day celebration will be entered into a drawing to win our prize package including a Princess Leia: Endor Adventures figure and a Captain Cassian Andor Black Series figure. Photos can feature just one person's celebration or something from the entire family. Be sure to enter by the end of the day on May 18th because the winner will be selected randomly and contacted via email on May 19th.

Complete the form below and read the photo release information to share your photos with us. Photos can be submitted using this form or by emailing them to the Children's Room staff at kids@adamslib.org.

This form was developed based on language used by our friends at the Carnegie Science Center, GSK, and Carnegie Library of Pittsburgh.
