Cheerleading Tryout Form 2019-20
Please fill out the following form in its entirety. This form serves as an agreement and your submission as an electronic signature.  

NOTE: ALL girls/boys are trying out to be cheerleaders at Pius.  You may be selected for Competition, Football, Basketball, or all of the above.

This form must be completed by April 20, 2019 to be eligible.

Agreement
I understand that by filling out this form I am committing to the squad placement I am given.  I GIVE MY WORD that I will not quit my squad because I do not like the outcome of tryouts.  I also understand that the decisions of the judges and coaches are final and that NO MEETINGS will be held to discuss the squad placements. I understand that failure to maintain membership on a squad prevents me from trying out for the program during the remainder of my years at St. Pius X.
Squad Choices
Please check all that apply. 9th and 10th graders are eligible for all upper levels and can check any of the boxes. If you have questions, please see the powerpoint on the cheer page regarding eligibility.

VERY IMPORTANT - If you are entering 9/10 grade - Cheerleaders who cheer on competition cheerleading likely CANNOT be placed on 9/JV Football Cheerleading. If you make VFB cheerleading then you can cheer both; otherwise, you need to list your preference in the comments so that we know what you would like to do if you have to decide. We are still working through the details on this point, but we are collecting the information now so that we have what we need after a final decision is made.

If you are a rising junior, are you willing to cheer on a JV level squad if you are not placed on a varsity level squad? *
(Saying 'yes' does not mean that you will be placed on JV just because you're willing to do it...it simply means that if you do not place on a Varsity squad this year then we will consider you for the other level.)

Competition Camp is June 20-22 and FB Spirit Camp is July 21-23.  BB does not have a scheduled camp time yet. I understand that failure to disclose an inability to attend will result in immediate removal from the squad.
Do you have any tumbling skills beyond a round-off that you would like to show? *
Tumbling adds to your score whether you are trying out for competition or not.
