Hello LSA and JSIS Families!

Please fill out the following form to join the waitlist for afterschool care for the school year 2023-2024. Although we would love to offer after school to any JSIS family who is interested, we are limited in space for now. Unfortunately we need to prioritize who can enroll and we start with students already in the program, followed by siblings, families who have special socio-economic needs and then first come-first serve.

Please complete the survey once for each child that is seeking childcare.

Note that this survey is not a commitment to provide childcare, and completing this survey in no way ensures care. Completing this survey will place you on a waitlist for the days you specify that you need childcare. We regularly have openings for one reason or another and use our waitlist to fill spots.

LSA will notify you of any changes and developments as they relate to your waitlist status.
