Please fill out this form if your student plans to participate in fall sports season.  This is NOT an interest form, this is the official registration form.  

All fall sports will start practice on August 30th.  Once the teams are set, information regarding practices will be sent to the emails you include in this registration form.  Fall schedules are posted on the Athletics page on our website.

FORMS REQUIRED FOR PARTICIPATION
All forms are to be turned in at your first practice.
Student-athletes may not participate without these forms.

3. Current Physical-- we understand the quick turn around time for these and encourage you to reach out to your doctor's office as soon as possible.

4. Student-Athlete Uniform Agreement

5. Participation Fee-- all HS sports have a participation fee of $125 per student and all MS sports have a participation fee of $75. Please wait to pay until the first day of practice if paying online.
