Thank you for providing us with this evaluation. It is a valuable part of the scholarship selection process. Your assessment of the student should address the applicant’s honesty, conscientiousness, ability to accept responsibility in life and/or in the classroom as well as any qualifications the student possesses that are pertinent to their preferred area of study. Any special qualities which are outstanding in the student, such as concern for others or kindness, should also be noted.

In addition to a completed Evaluation Form, a letter of recommendation on official school letterhead, may be provided. (Not Required).
