Please only use this Form to submit a Health Report or cause of death if you are a UK St Bernard breeder or owner.

You can use this Form to enter a Health Report for ONE St Bernard (born any time from 2009).  Please submit a new Form for each additional St Bernard.

By using this Form you agree to the St Bernard Health Committee using the data you supply to inform their Health Plans and to provide reports on the health status of UK St Bernards.

Your personal details and those of your dog(s) will not be shared publicly and will only be used, in confidence, by the Health Committee. We may share anonymised data with our research partners such as the KC, AHT and RVC for further analysis.

Remember to let your dog's breeder know about any serious health conditions.
