All of the details that are submitted are confidential for your medical records and will not be shared with any other parties. We realise that the questions are lengthy but this eliminates the need for paperwork to be filled in at the practice which is very important at this time.
We ask at this time that  if possible patients attend the practice on their own. If a parent or carer is required we ask that this person fills in a covid screening update to confirm they have no covid symptoms and for trace and protect.
