I am requesting to be absent or tardy from practice on the date(s) above. I understand that filling out this form is not a guarantee that the absence will be excused. Attendance is crucial to the success of the whole team and missed practice places complications and difficulties on the entire team. I understand that any unexcused absences or continual excused absences can result in being placed on probation, being replaced, moved to a different team, or removed from any team.

Absent request forms will only be accepted 30 days prior to the absence and none will be accepted within the 30 days. All absent requests will be approved or unapproved by the coaches and/or Cheer Director STRICTLY based on the guidelines stated in the 2022-2023 season contract. All unexcused absences will result in a $40 UNEXCUSED ABSENCE FEE to your account. Absolutely NO practice can be missed within two weeks of a competition. Please review athlete contract for details.
