Aftercare Application
Aftercare is from 2:30-6:00 PM while school is in session (12:45 PM on Mondays). Payments must be made the week before service, on time, and in FULL for child(ren) to attend. 

Fees: $25.00 Registration Fee per Child due at enrollment. 
$40.00 Weekly Fee for Aftercare due the Friday before the week of service. 
$5.00 Late Fee for 1-5 minutes after 6:00 PM + $2.00 per minute after 6:05 PM. 

Parent Pick up time: 5:45 PM Parent's are considered late after 6:00 PM at which time they will be charged late fees. 

There will be NO busing home from Aftercare. All scholars will have to be picked up from school promptly at 6 pm.

I understand that my child can be suspended or dismissed from the program if his/her behavior is not acceptable. I agree to pay all fees in full and on time. I understand that my child will not be allowed to attend if fees are not current and all fees are due the Friday before the week of service. There are no daily fees; if my child attends one or more days I agree to pay the $40 weekly fee.
