REHEARSALS---I understand that my child is expected to attend all studio rehearsals at BWCAS in Berea, and all tech rehearsals at the theater in which my child will perform. Any potential conflicts MUST be discussed with the director as soon as possible. I understand that my child IS NOT EXPECTED TO ATTEND ANY REHEARSAL WHILE PRESENTING ANY COVID SYMPTOMS OR OTHER ILLNESS. *

WAIVER----I acknowledge and accept risk of illness and injury associated with participation and agree to hold harmless North Pointe Ballet and its affiliates. I agree that North Pointe Ballet and its affiliates have taken reasonable measures to ensure my safety. I authorize NPB and its affiliates to use my dancer's image and likeness in whatever way they see fit. *

COVID PROTOCOLS---Masks will be required for every dancer of all ages attending any indoor NPB rehearsals. For performances, dancers will remove masks immediately before taking the stage and will put their mask back on as soon as they exit the stage. NPB will continue to comply with government, state, and local agencies as well as follow best practices and industry standards regarding CoVID protocols to keep all dancers safe. I have read and understand the protocols outlined in the cast only page. I understand that any dancer who is potentially exposed or exhibits symptoms will not be allowed to rehearse or perform until it is deemed safe by county health department guidelines. If any dancer is notified of a potential exposure, he/she will notify Melaina Kampf at melaina@northpointeballet.org. *

PARTICIPATION FEE---I understand there is a participation fee varying between $95-$165 depending on my child's role. I am also responsible for purchasing tights (approx $12) and shoes (approx $24) through NPB. (Additional info on cast only page) *
