MadWorC Membership Application/Renewal for 2024

Co-ops wishing to be members of MadWorC in 2024 must fill out this form. If you want to seat someone on the MadWorC board, you must fill out this form by January 16, 2024.

Questions with a * are required, other questions are optional, but it does help our community learn how we can best support each other and the growth of more worker cooperatives in Madison to have information about these things.
