COVID-19 Waiver & Release of Liability
Due to the outbreak of COVID-19, feis organizers are taking precautions for the safety of feis attendees at the 2021 Magnolia State Feis and Feis New Orleans & Louisiana State Championships (referred to as "feis" herein collectively, whether one or both). Please initial or sign below to indicate that you have read and understood the Waiver and Release of Liability, and affirm that you are not suffering from any COVID-19 related symptoms or are waiting for COVID-19 test results before entering the feis location.

Symptoms of COVID-19 include: fever above 99 degrees Fahrenheit, loss of taste or smell, fatigue, new onset of severe headache, chills or shaking with chills, congestion or runny nose, significant muscle pain or body aches, uncontrolled cough, difficulty breathing, or diarrhea, vomiting or abdominal pain.

By signing this Waiver and Release of Liability below, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to COVID-19 or infected while attending the feis and that such exposure or infection may result in personal injury, illness, disability, and death. Despite efforts to undertake social distancing and safety measures for COVID-19, I understand that the risk I am undertaking may result from the actions, omissions negligence or gross negligence of myself and others, including, but not limited to the Ryan School of Irish Dance, the Hilton Airport New Orleans Hotel, feis attendees and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself for personal injury (including, but not limited to, illness, disability, and death), damages, losses, claims, liability, or expenses of any kind, that I may experience or incur in connection with attendance at the feis. I hereby agree to release, waive, discharge, hold harmless and covenant not to sue the Ryan School of Irish Dance, the Hilton Airport New Orleans Hotel and their directors, agents, employees, and representatives, feis attendees, and their families of and from any and all claims, including any injury to myself for personal injury (including, but not limited to, illness, diability, and death), damages, losses, claims, liability, or expenses of any kind.
