This survey takes 5 - 10 minutes to complete.

The survey has three parts.

The first part deals with wellbeing outcomes and asks a key question (seven questions in total) relating to each component of DVA's Wellbeing Model being:

   1. Recognition and Respect
   2. Health
   3. Education & Skills
   4. Housing
   5. Social Support & Connection
   6. Employment
   7. Income & Finance

The second part has two questions relating to access to services and support.

The final part of the survey has a few general questions about your service background and your affiliation with ex-service organisations.

Responses are anonymous and we will not publish any information that could be attributed to any individual.

There is an option at the end of the survey to provide us with your email address if you would like us to get in touch.

Australian Veteran News (AVN) will publish the results as part of our effort to inform ex-service organisations and government agencies on what they can do to better serve the veteran community.
