In consideration of being permitted to practice, compete, officiate, observe, work,
volunteer or participate in the Delta-Greely Skating Association (DGSA) on or off-ice
activity during the 2020-2021 season, I , for myself, family and my immediate
household (including minor children), agree to the following:

• I/We understand attending sporting activities means an increased risk that my
player family and members of my immediate household may be infected with
COVID-19.

• At no time will I/we participate in or attend a DGSA activity if any member of my
household exhibits any COVID-19 symptoms or tests positive for COVID-19. I/we understand that if any member of my household tests positive I/we may return once quarantining according to State of Alaska guidelines is complete or all members present a negative test result to DGSA COVID-19 representative Austine Martin.

I am familiar with DGSA mitigation plans, CDC, state and local guidelines.

• I/We agree to notify a DGSA Board Member or Coach if any member of my family and
immediate household is diagnosed with COVID-19.

• I/We acknowledge that I/we are aware that by participating in any DGSA event that there
are inherent risks to me, my children, family and members of my immediate household of
being exposed to COVID-19 and/or any mutation or variation thereof, and that I am also
aware that such an exposure can occur directly or indirectly.

• I understand that while DGSA strives to provide safe on and off-ice activities, DGSA cannot
eliminate all risks. By signing this Waiver of Liability, I/we assume all risks on behalf of
myself, family and immediate household, including minor children, regarding the possibility
of contracting the novel corona virus known as COVID-119, or any mutation or variation
thereof.

• I/We hereby voluntarily agree to RELEASE, WAIVE, DISCHARGE, and COVENANT NOT TO
SUE DGSA or Friends of DGSA and/or its officers, directors, and employees; the Alaska
State Hockey Association; and any individuals companies, or associations having anything
to do with DGSA operations, including but not limited to participants, officials, and owners
of the premises where DGSA operates on or off ice activities (collectively referred to as
RELEASEES). By signing this document, RELEASEES WILL BE RELEASED FROM ANY
AND ALL LIABILITY for any and all loss or damage whether caused by the negligence of
the Releasees or otherwise related to possible exposure to or contracting of the novel
corner virus known as COVID-19 or any mutation or variation thereof; and

• I hereby agree to IDEMNIFY AND HOLD HARMLESS the Releasees from any loss, liability,
damage or cost (including reasonable attorney’s fees) they may incur arising out of or
related to my illness or death, or the illness or death of my children, family or immediate
household, including the actual participants(s) in DGSA activities whether caused by the
negligence of the Releasees or otherwise.

Any of the above may be changed in an effort to promote a safer environment with or without
notice as seen fit by DGSA.

Failure to abide by any of the above may result in expulsion from DGSA for the 2020-2021
hockey season with loss of registration fees.

BY COMPLETING THIS FORM, YOU AGREE YOU HAVE READ AND UNDERSTAND THIS WAIVER AND
RELEASE AND AGREE TO BE BOUND BY ITS TERMS.
