River Tree Arts Consent Form
Please fill out this consent form for River Tree Art's 2023 Summer Camp season. If you fill out the digital form, you will not need to fill out a physical copy. This form helps us to gather all medical and emergency contact information so that our faculty and staff can ensure a fun and safe summer experience. I agree if any allergies or medical conditions are listed above, the Parent/Guardian is responsible for communicating allergen / medical conditions to River Tree Staff prior to the first day or class. An emergency medical plan of action to follow in the even of an accident/emergency should be emailed to frontdesk@rivertreearts.org *
All students under the ages of 14 years must be signed
in and out of class by someone authorized by the Parent/Guardian. Please provide the name and relation of people allowed to pick up your child(ren) in your absence.

Example: Jane Smith (Family Friend) 
River Tree Arts staff takes pictures, slides, and videos of participants in the various activities for use in the marketing and promotion of our programs. I hereby grant permission to River Tree Arts to use the pictures, slides, and videos of me and/or my child(ren) for this stated purpose.

Offsite Consent: I give permission for my child(ren) to participate in free time outside of the classroom at Lower Village Park located at 159 Port Road (off Route 35), Lower Village, Kennebunk , ME 04043 under the supervision of River Tree Arts staff at all times.

If a faculty elects to bring students to the park, the group will be accompanied by an additional adult as well as a first aid kit

Pick up/Drop off Policy: Parents are to drop off students no earlier than five minutes before camp begins and pick up students no later than ten minutes after camp ends

Parents must accompany children into the building to sign their children in at the beginning of camp and to sign out when they leave.

911 Policy: In all cases and circumstances, River Tree Staff will call 911 and the staff will abide by and follow all EMS/911 instructions during a medical emergency concerning the above mentioned student. The Parent/Guardian will be notified by River Tree Arts staff at the earliest opportunity

Withdrawal and Refunds:
You may withdraw from a course 14+ days prior to the first class and receive a 100% refund. You will receive a 50% refund if you withdraw within 2 weeks of class starting. If you withdraw within 1 week prior to the first class, no refund will be given. There are no refunds once a class begins. If River Tree Arts cancels a course, you will receive a 100% refund in the same form as the original payment was made or choose to be issued a credit towards a future class (credit must be used within one year of issue). River Tree's Withdrawal and Refund policy will apply to cancellations due to illness or quarantine.

Cancellations:
River Tree Arts reserves the right to cancel any camps or classes that do not have a minimum number of 4 students, depending on the
course being offered. Students will be notified at least 72 hours in advance. If River Tree Arts cancels a course, you will receive a 100%
refund in the same form as the original payment was made.

If RSU 21 closes due to inclement weather, all group classes and private music lessons will be canceled. If RSU 21 has early dismissal, there
will be no after school classes or private lessons. Make ups will be scheduled by the end of the session.

River Tree Arts reserves the right to cancel courses, substitute instructors, change course times, and limit enrollment. Every attempt will be made to run after school classes in the event the primary instructor is not available: this includes substitute teachers and/or changing
the class theme for the day.

Allergies:
Our student’s safety is one of our highest priorities. The Parent/Guardian is responsible for communicating allergen information to River Tree staff at the time of enrollment and registration, prior to the start of class.

If your child has any food allergies, please provide River Tree with an emergency medical plan of action to follow in the event of an accident / emergency. Students with food allergies should bring their own snacks and lunches.

River Tree Arts is a nut-free facility. Please refrain from sending in snacks or lunches that contain nut products.

Medical Conditions:
We understand that some campers have medical conditions so please notify us at the time of registration to ensure we can provide everyone with a safe and fun experience.
