1)If you require ICT support, do complete and submit this form and the ICT Team will get back to you during
school hours.
2) Form submitted outside school hours will be attended the next working day.
3) It's important to include correct location details and brief description of fault / issue.
4) Proper completion of form will facilitate & expedite processing of repairs/maintenance.
5) Please do inform us at least 3 days in advance for internal, external exam or any event setup.
6) Please do inform us at least 1 day in advance for loaning of ICT/AV equipment.
