This form is meant to capture a listing of skills and experience for candidates to be considered for Board membership. To properly fill out this form, you'll need to have the following details ready for submission: addresses, contact information, and current occupation. Furthermore, you'll be asked to rate your your skills and experience related to the following fields/sectors: business, community, finance, governance, innovation & technology, post secondary education, and any other applicable areas. Additionally, there are sections to outline your specific attributes for consideration. Lastly, there is a volunteer self-identification section to better understand Board diversity based on lived experience.
