Harmony School of Innovation is providing technology and live online instruction, so that our students can participate in engaging lessons from the comfort of their own homes. Students will join their scheduled live sessions to learn from their teacher, collaborate with peers, and receive immediate educational support. We are helping our students stay connected by lending chromebooks to every student who does not have a personal device at home. Please complete the following survey and forms to indicate your child's technology needs.

Chromebooks and instructional materials for Harmony School of Innovation students will be distributed in driveline on the following days. Please select the day and times that you will pick up the chromebooks(s) for your child(ren). Please also note that the first day of school is Monday, August 17th, so all technology must be picked up from the school no later than Friday, August 14th at 4:00 pm. *

Please check the following boxes to indicate that you understand and agree to the following HPS technology guidelines. Chromebooks will not be given to students without agreeing to all of these terms. (All boxes must be checked "YES" to receive a chromebook) *

YES : As the parent or legal guardian of the minor/student signing above, I grant permission for my child to access Harmony Public Schools technology resources, including Internet accessibility and my child’s assigned Chromebook. I understand that my child, or the child in my care, may keep his/her network access and Chromebook as long as the procedures and rules described in the Harmony Public Schools Acceptable Use Policy and the Harmony Public Schools Chromebook Student/Parent Agreement are followed. Should my son or daughter, or the child in my care, violate any of the previously cited rules or procedures, they will be held accountable for their actions by Harmony Public Schools.

YES : I fully understand the costs and responsibilities associated with the Harmony Public Schools Chromebook Student/Parent Agreement. I assume full financial responsibility ($315) for theft or damage. Failure to turn in a Chromebook will result in the student being charged the full current market value of $315 for a new one.

YES : Return the Chromebook, Accessories and Case to Harmony Public Schools at the end of the closure or last week of 2019-2020 school year (whichever occurs first). Students who withdraw enrollment for any reason must return the Chromebook on or before the date of withdrawal.

YES: ELECTRONIC SIGNATURE AGREEMENT AND CONSENT: I hereby authorize Harmony Public Schools to accept this correspondence and documents transmitted by me via electronic means. In addition, I certify that these documents are made “upon my signature” and acceptable as replacement for my written signature. I understand that the electronic signature may not be denied legal effect or enforceability solely because it is in electronic form. I understand that if I choose not to accept this electronic communication and use of electronic signature, it is my responsibility to manually sign and deliver the documents to Harmony Public Schools.
