PARENTAL CONSENT FOR AN EDUCATIONAL VISIT

CHALFONT ST GILES JUNIOR SCHOOL – YEAR 6

Year 6 Games Festival at Dr Challoner's High School, Cokes Lane, Little Chalfont, HP7 9QB.

On Tuesday 27th June 2023 from 9.00 to 15.00 (Event time 9:30 to 14:30)

All of the children in Year 6 will be participating in a Year 6 Games Festival at Dr Challoner's High School on Tuesday 27th June between 9.00 and 15.00 (event time is from 9.30 to 14.30). The children will be travelling to and from the festival by coach.

Children will need to bring in a packed lunch to eat at the festival.

Children should come in to school wearing their school PE kit (green T-shirt, black shorts or black jogging bottoms and Leavers hoodie) and trainers.

Children will also need a named water bottle to take with them.

We will need some parents to help on this trip. if you would like to help and have a current school DBS please contact the School Office.

By completing this form I am giving consent for my child to attend the Year 6 PE Games Festival at Dr Challoner's High School on Tuesday 27th June. I confirm that I have read the relevant information above. Consent forms to be completed and returned by Monday 12th June.

I agree to their participation in the activities described and I acknowledge the need for and expect them to behave responsibly. I can confirm that the school has current contact and medical details for my child.
