If you are volunteering in person with the American Canyon Middle School PTO, we must have a Volunteer Waiver and Release Form on record for every fundraiser and event you volunteer for.
Note... We can only accept one person per form. For instance, if you and your spouse are volunteering, we will need a completed form for each of you.
