This form should be completed by parents on behalf of their children in grades K-5. Students in grades 6-12 may complete this form on their own behalf.
Prior to submitting the form each week, it might be a good idea to write down the activities your children are signed up for - technology is great, but does have some struggles from time to time. While sign-ups will be on a weekly basis, you can still view enrichment activities for the whole month (as they're updated) by

* If your plans deviate from what you've indicated on this form, PLEASE inform the district (either contact the Director of Special Programs, Brianna Sick, or the Elementary Office Secretary, Kelliann Abbott).
This ensures that our records are accurate and students are accounted for.
There may be some capacity limits on some of the activities, so if you notice that an activity is no longer appearing as available, that's because the capacity limit has been reached; please select another activity. Lastly, please only sign up for the activity if it aligns with your child's grade level; if the activity is only offered for 3rd grade, and your child is in 1st, please don't sign them up. Doing so takes that spot away from someone else.
Enrichment activities run from approximately 3:40-4:50PM unless otherwise noted. This allows students to grab an afternoon snack from the cafeteria before the activity begins. Students needing transportation home can board the 5PM Late Bus; just be sure to indicate your child's transportation needs below so that we can plan accordingly. Students using the Late Bus at 5PM will be dropped between 5:05-6:15PM.
