To be considered, this form must be filled out no later than March 15, 2021. IMPORTANT: If the blank has a * it is a required response.

TO COMPLETE THIS APPLICATION YOU WILL NEED TO

1. Completed this form in its entirety.
2. Submit an administration fee of $100 online. (The $100 fee is a portion of the total Nexus fee) You will receive an email from USA Field Hockey to pay the $100 fee. The fee is not included in this form. (No Checks will be accepted. Registration and fee must be submitted online).
3. Include a Personal Statement of Need (to include reasons for needing financial aid).
4. Email a copy of your 2020 Federal Income Tax Return to nexus@usafieldhockey.com.
*Financial aid is allocated based on the number of athletes that are approved and the total dollar amount assigned to the Nexus program for the year.
