PH 21-22 Bus Program Request a Seat Form

The initial bus program sign up has taken place. Now as seats on a bus open up we will fill them from names submitted on this form.

If a seat opens up on the route you request you will be notified by email (the one you enter in this form) that a seat is available for your student.

Only a parent/guardian who is named in our Skyward system for the student is allowed to complete this sign up form.

The parent/guardian is allowed to only sign up their own children - students who are listed in Skyward for them.

Submitting this form DOES NOT mean your student has a spot on a bus, rather it is putting in a request for the student for when a seat opens up on a route (waiting list).
