Any student wishing to serve at Westminster must have a parent’s permission to do so. Both the student and the parent need to read and agree to abide by the following guidelines. By completing and submitting this form, you are giving your student permission to serve at Westminster. A separate form will be sent to your student and must be completed by them prior to their service.
