Use this form to request for an Investigator or approval for an Expert Witness on your appointed case. Once approved, if you need additional hours for your case, please submit additional requests on this form. Please submit one per case and type of request. If you need additional hours, submit form again.

If your hired expert needs to be paid have them send the invoice to billing@lpdo.org. They will need to send their W-9 and a direct deposit slip so we can issue payment to them.

Note* If you estimate that the cost of services covered by this request will exceed $1500, you must obtain an order from the court prior to submitting this request.
