In June 2021, we invited the congregation to complete a survey about your comfort levels around indoor worship. As part of that survey you also were invited to report the vaccination status of all members of your household.

This survey invites you to update your vaccination status. We are aware that many in the congregation that are eligible have received booster shots, and that many children ages 5 and up who were not eligible for vaccination in June are now vaccinated.

For this reason, we invite each household in the congregation to complete this brief survey BY MONDAY DECEMBER 27, 2021.
