CCSS Hotspot Request Form
Use of a hotspot is subject to the terms and conditions set forth in this checkout/access agreement, and by checking out the hotspot, you have agreed to the following:
I understand that the hotspot can only be checked out by a parent within the Chickasaw City School District (CCS). All students must have a signed Device Use agreement on file before a hotspot can be checked out.
I understand that my student(s) computer usage will be monitored by CCS. Any access of “inappropriate matter,” meaning pornographic or indecent material as defined in the device use agreement, will violate the agreement.
I accept responsibility for all items that are checked out and will return them in the same condition as they were received. I will ensure the safe and timely return of the equipment within the loan period.
I understand that the hotspot runs on the T-Mobile mobile networks and that the speed and availability of the Wi-Fi connection will be dependent on the service area of T-Mobile towers. Service connection is not guaranteed in all areas at all times.
I understand that CCS is not responsible for any files, data, or personal information accessed, transmitted, lost or damaged while accessing the Internet via the hotspot.
I understand that T-Mobile and Chickasaw City Schools will only provide 100 GBs of data at no cost to families and that this device will be used primarily for educational purposes.
By completing this form, I accept the above checkout agreement and am stating that I am responsible for returning this equipment to CCS in good working condition and free from damage.
