Please read the following form carefully, there are 3 sections.  Before you complete the form you should have read all the Return to Swimming Guidance and FAQ's sent to you.

Section 1 - Covid-19 Health Survey
Section 2 - Covid-19 Code of Conduct
Section 3 - Covid-19 Declaration

As per the guidance from Swim England, in order to return to swimming we will need you to respond to all the required questions.  

Please complete one form for each swimmer.  Swimmers over 18 must complete the form themselves.

You will receive a confirmation email on completion of the form for each swimmer.
