This form seeks your consent for your child’s participation in general excursions and sporting activities and for the College to arrange medical treatment for your child while on an excursion should it become necessary. You will be advised as soon as practicable of action taken in relation to medical treatment for your child.

Prior to each excursion, a notice will be sent out containing additional information in relation to the particular excursion.  

Throughout the year your child may be involved in the following off-campus activities:

ALL SCHOOL
• Athletics Carnival - HSC Glendale
• Cross Country - College surrounds
• Swimming Carnival– The Forum

JUNIOR SCHOOL
• Gymnastics Lessons K-6 - Glendale
• Swimming Lessons K-6 – The Forum      

SENIOR SCHOOL
• Gymnastics - HSC Glendale
• Koombahla Nursing Home - Elermore Vale
• Year 7 Swimming Lessons – The Forum
• Year 9 Indoor Cricket - Howzat, Newcastle    

Non-attendance at an excursion:
If your child cannot attend an excursion due to illness or for any other reason, please notify the College immediately.  
Non-attendance will require an absence note.

Insurance:
The College’s insurance through AIG provides cover for excursions.  Please refer to the College Handbook for additional policy information.
