High School Seniors: Your uploaded transcripts must include your senior grades for 1st semester. We will not have fall grades available until early February.  This form serves as instruction and permission for CHS to send your transcript.
The deadline for OSAC to receive transmitted transcripts is 2/15/24 (Early Bird Review and drawing for a $500 scholarship) or 4/1/24 (final deadline to qualify for any OSAC scholarship- no exceptions.  This includes CCHS Alumni Scholarships, which have an additional application due later in the spring).
