ACADEMY WOLVES - Student-Athlete Interest Form (2022-2023)
Dear Academy and SOTA Students -

If you are interested in participating in sports next year, you will need to fill out this survey. The athletic department will follow up with any additional information regarding next steps. We welcome all students on our teams and expect good communication, attendance, and sportsmanship from all players.

Paperwork must be completed before your first practice; the paperwork includes the Physical Exam, Parental Consent, Steroid Info, Concussion Info, Sudden Cardiac Arrest Info, Heat Illness Info, and Opioid Info forms.

* Additional information, student resources, and additional safety information will be circulated in the next few months via SFUSD email.
