2024 Diploma Form
A student's diploma should include a student's full legal name. Please provide South Kent School with your student's full legal name (first, middle, and last) and type out how you would like it to appear on his diploma. 

We would also like to collect your student's "hometown" for our Prize Day Program. If your student is from the United States, please provide a city and state as their hometown. If you have an international student, please provide a state/province/prefecture/region and country as their hometown. 

If you have more than one graduating student in 2024, please fill out this form twice. Please be sure to double-check that everything is spelled and typed out correctly before submitting this form.
