RC LTR Registration Form Fall 2021
Please fill out all of the requested information to register your son/daughter for a 2 day (4 hours per day) Learn-to-Row (LTR) session. We are offering 3 sessions in 2021 Fall:

LTR 1: Sept 11, 12:  8am - 12 noon (Closed)
LTR 2: Sept 18, 19:  8am - 12 noon (Closed)
LTR 3: Sept 25, 26: 8am - 12 noon (Closed)

Each session is $300.

All potential rowers must pass a swim test and submit the swim test form to ridgewoodcrew2k@gmail.com. The swim test form can be found here: http://www.ridgewoodcrew.com/uploads/4/7/6/4/47643011/rc_swim_test_certification_form_april_2019.pdf 

**IMPORTANT** Please read and accept the Ridgewood Crew COVID-19 Safety Protocol checklist that is part of this form. Please note that the rower's and coach's health and safety is of utmost importance and we will follow the state and local health guidelines.

Payment: Please mail a check (made out to Ridgewood Crew) to the address below or pay via Venmo (@RidgewoodCrew, code: 8698 if needed).  Please note that your son/daughter will not be able to participate till the payment is received.

I, the parent/guardian of the above-named child, hereby give my full consent and approval to his/her registration and participation in Ridgewood Crew for the 2021 calendar year. I understand that there are certain risks of injury inherent in the practice and races of crew, as well as transporting my child to or from any of the regattas or crew-related events, and I am willing to assume all risks and hazards incidental to the conduct of such activities. *
I hereby confirm that my child is fully capable of participating in the sport of crew and that my child is in sound medical condition and has no physical or mental disabilities or conditions that would restrict full participation, except as noted below.  Please include all known allergies. In the event of my absence, if my son/daughter is suffering from an injury or condition requiring diagnosis and immediate medical or surgical treatment, I do hereby voluntarily authorize Ridgewood Crew coaches, Board members, and/or parent volunteers at the scene to consent to such diagnostic procedures, medical/hospital care, medication administration, and to such medical, surgical, or x-ray treatment as may be deemed necessary by and under the direct authorization of a licensed physician/practitioner.   *

In addition, I hereby release, absolve, and discharge Ridgewood Crew and its officers, directors, coaches, sponsors, supervisors, representatives, and any person transporting my child to and from any of the activities from any claim or liability in case of an accident, injury and/or medical and dental expenses incurred to my child. *
Terms and Conditions: 1. A one-time swim test certification is required for all Learn-To-Row students/first-time rowers and can be completed at a YMCA or by a certified lifeguard.  The Swim Test Certification Form can be found at http://www.ridgewoodcrew.com/forms.html    2. All programs must be prepaid, with a check payable to Ridgewood Crew or Venmo.  Receipt of full payment is required for participation confirmation.  3. NO REFUND, NO prorating of fees, NO switching of sessions and NO credits will be provided for athletes who drop out of the program. If you miss a session, a make-up practice is allowed once, subject to prior arrangement with the coach(es). 4. Acceptance of the RC COVID-19 Safety Protocol (details below;) is required prior to attending the summer LTR program. The full safety protocol can be found here: http://www.ridgewoodcrew.com/  5. I give my full consent for my child to row in a multi-person (other than his/her siblings) and absolve Ridgewood Crew from any liability. *
