Please fill out all of the requested information to register your son/daughter for a 2 day (4 hours per day) Learn-to-Row (LTR) session. We are offering 3 sessions in 2021 Fall:

LTR 1: Sept 11, 12: 8am - 12 noon (Closed)
LTR 2: Sept 18, 19: 8am - 12 noon (Closed)
LTR 3: Sept 25, 26: 8am - 12 noon (Closed)

Each session is $300.

**IMPORTANT** Please read and accept the Ridgewood Crew COVID-19 Safety Protocol checklist that is part of this form. Please note that the rower's and coach's health and safety is of utmost importance and we will follow the state and local health guidelines.

Payment: Please mail a check (made out to Ridgewood Crew) to the address below or pay via Venmo (@RidgewoodCrew, code: 8698 if needed). Please note that your son/daughter will not be able to participate till the payment is received.

Terms and Conditions: 1. A one-time swim test certification is required for all Learn-To-Row students/first-time rowers and can be completed at a YMCA or by a certified lifeguard. The Swim Test Certification Form can be found at http://www.ridgewoodcrew.com/forms.html 2. All programs must be prepaid, with a check payable to Ridgewood Crew or Venmo. Receipt of full payment is required for participation confirmation. 3. NO REFUND, NO prorating of fees, NO switching of sessions and NO credits will be provided for athletes who drop out of the program. If you miss a session, a make-up practice is allowed once, subject to prior arrangement with the coach(es). 4. Acceptance of the RC COVID-19 Safety Protocol (details below;) is required prior to attending the summer LTR program. The full safety protocol can be found here: http://www.ridgewoodcrew.com/ 5. I give my full consent for my child to row in a multi-person (other than his/her siblings) and absolve Ridgewood Crew from any liability. *

RIDGEWOOD CREW COVID-19 SAFETY PROTOCOL: Core Principles - Check that you have read and agree to each item. *

1. Athletes must follow the rules and regulations set forth by the CDC and the NJ Department of Health. Training should focus on a 'get in, train, get out' approach, minimizing unnecessary contact.

2. All rowers and coaches should sanitize or wash their hands with soap for at least 20 seconds upon arrival and should follow hand-washing protocols regularly. Athletes shall carry small containers of hand sanitizer to use after touching potentially contaminated surfaces.

RIDGEWOOD CREW COVID-19 SAFETY PROTOCOL: Athlete Health Monitoring/Return to Training Guidelines - Check that you have read and agree to each item. *

1. Any individual (athletes, coaches, and staff) returning to the boathouse or to a training needs to have been in that general geographic location for 14 days prior to coming tot he pen.

3. All individuals must self-monitor for symptoms of COVID-19 daily. These symptoms include: fever, chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea. If any of these symptoms exist the person should not come to the pen.

3. If individuals feel any of the symptoms mentioned in #2 during or after practice, contact the coaches and/or Ridgewood Crew Board member and do not attend practice. The individual should reach out to his or her doctor for assistance.

4. If an individual has had a documented case of COVID-19, they will need a note from their doctor indicating they are cleared to resume participation in training.

RIDGEWOOD CREW COVID-19 SAFETY PROTOCOL: Outdoor Facility Use Guidelines - Check that you have read and agree to each item. *

1. Ridgewood Crew will follow state and local guidelines pertaining to parks and public waterways in the operation of on-the-water training.

2. Any shared equipment that is being used, will be thoroughly cleaned and disinfected with alcohol (70% minimum) disposable surface wipes before and after each use.
