Thanks for being a voter! It is too late for us to *mail* a ballot request to you.

HOWEVER, you can still request a ballot be mailed to you by taking the following steps:

Write your:
- Drivers license number or State ID number
- Full name
- Date of birth
- Mailing address
- What ballot you want (example: Democratic Primary ballot, CD 1 Special election ballot, or both if you are in CD1)
- SIGN and date.

Then take a picture and email it to your county clerk, or drop it in the drop box outside their office.

**The Deadline for requests is April 29 at 6pm.**
