By completing  this form you will become a member of Young Hackney. This means you can attend any of our centres and take part in our activities and projects.

Young Hackney staff can provide you with information, advice and guidance on a range of topics – for example, about your health, about careers or about education. And we can help you with any difficulties you are having, whether this is at school, home, or with friends. In order to do this we need to hold some information about you.

Why do we need to keep information about you?

This form will go with you to the hospital should you fall ill or have an accident while with us
It will help doctors to treat you if you fall ill or have an accident
So we can contact someone from your family if you have an accident or there is an emergency
It helps us to provide the best service that we can
It helps us to follow your progress and be there for you if you need us
So we can make sure you can get the services of trusted organisations if you need more or different kinds of support


What information do we keep about you?

* Your name, gender, date of birth, ethnicity, address, phone number and school/college details.
* A register of activities and events you have attended.
*We may also keep notes of discussions that describe what you are seeing us about.

We keep this information on a secure file. You have the right to see the information that is kept about you.

When will we share information with other people?

The Data Protection Act 1998 is the law that governs what information we keep, how we keep it and who we can pass it on to. The Data Protection Act is very clear about only sharing information when it is necessary. So, for example, we may:

* Talk to another professional in the service about your needs to develop an appropriate support package
* Talk to a college/training provider or employer about you to help you get an interview
* Talk to another agency (for example your school) that we are working with to help meet your needs

There are a few situations when we would have to tell someone, even without your permission. These are:
* To prevent a crime taking place, either now or in the future
* If there is a risk of harm to you or others

If you need help filling in this form, please ask a member of staff. If you are under-16 we need to know if your parent(s) or carer(s) are happy for you to be a member of Young Hackney. They will also have to consent to data and information sharing.
